Tourist Visa :

• 1 Visa form & Affidavit (Only in case of multiple entry) duly filled and signed
• 1 Photo (35x45mm with 70 to 80 % face to be visible, white back ground- Matt Finish only)
• Overseas Mediclaim policy copy
• Air tickets (Original)
• Hotel confirmation directly from Czech Republic
• Covering letter explaining the reason of travel with name & designation of the signatory
• Proof of income i.e. last 3 years I-T Challan or TDS Certificate
• Latest bank statement
• Salary Certificate

Business Visa:

• 1 Visa form & Affidavit (Only in case of multiple entry) duly filled and signed
• 1 Photo (35x45mm with 70 to 80 % face to be visible)

• Air tickets (Original)
• Hotel confirmation directly from Czech Republic (compulsory, even if there is an invitation letter attached with the application)
• Covering letter explaining the reason of travel with name & designation of the signatory
• Proof of income i.e. last 3 years I-T Challan or TDS Certificate
• Latest bank statement
• Salary Certificate
• Invitation addressed to the Consulate of the Czech Republic in Mumbai to be submitted along with the case. Also a clearance letter from the Czec Police in ORIGINAL (Only for those pax who does'nt have a VALID UK/USA visa) is a must.

Submission Timings: 0900 hrs to 1000 hrs.

Collection Timings:   

1330 hrs to 1430 hrs (5 working days)

Fees :

Single/Multiple Enty Rs.4000 /- (90days validity) & Multiple more than 90days Rs.7000/-
